ALL of the following forms are required for camp. Please ensure that you fill out each form or your camper may not be properly registered.

• 2025 Camp Waiver (ID# 92385)
• Ridgecrest Conference Center Waiver (print and give to Troy Puryear)
• 2025 Gwathmey Medical Release
• 2025 Gwathmey Transportation Form

If you have completed the GBC medical release & transportation form since 1/1/25, you do not need to do this again.

• CHAPERONES ONLY:
  • Gwathmey Background Check Form – OR
  • Background Check Renewal (For returning Adult Leaders)
  • M-Fuge Child Protection Training Link

IMPORTANT MEETINGS: 

• Parent Meeting – June 1st at 3:00. This will be a time for us to go over with the parents some expectations we have, what the students can expect, answer any questions you have, and make sure we have all the forms we need.
• MFUGE Student Boot Camp – June 1st at 4:00. We want to take this time to give all the students a chance to meet everyone and talk about our expectations with them.  We can also let them know what to expect as well as answer their questions.
